Catchment Area
The Practice welcomes new patients who live within our practice area.
Any new patient wishing to register with our Practice must live within the Practice catchment area. You will be asked to complete a new patient application form.
A patient registering at the practice is asked to come to reception to collect our new patient registration forms.
When registering please provide:
- Photographic ID, Passport or Drivers Liscence etc.
- Proof of Address such as a Utility bill
This is all checked by our reception team before accepting. It is always helpful if a patient can provide a copy of any repeat medication they are prescribed for our records.
Our Practice covers all BD12 and some parts of BD6, BD19, HX3 and HD6. Please call at the surgery to look at a map of the area for further details.
Medical treatment is available from the date of registration. Please contact reception for further information.
